Helping ensure you're getting a "Form 1823" filled out by a physician on behalf of the senior in advance of touring any facilities (has a 60-day shelf life before a new one is needed). Download Form 1823 (AHCA, 2021).

• Identify potential eligible facilities that meet the senior's specific needs
• Schedule appointments with the facilities on behalf of the senior
• Meet the senior at the short-list of possible facilities, joining them on the tour and interview process with the facility

• Joining the senior at the established move-in date at the chosen facility
• Regular check-ins with the senior and the facility to ensure services are being met
