Senior living type assessment

  • Understanding the senior's personal needs to determine which type of care is needed (no care needed, private residence with at-home care, independent living, assisted living, memory care, skilled nursing)
  • What is important to the senior a year from now, 3 years, 5 years, or 10+ years

Financial assessment

  • What is the senior's budget
  • What is the senior's plan for payment
  • Explain the various payment options based on the type of care needed
  • Reviewing the possible payment options based on the specific senior's profile

Health assessment

Helping ensure you're getting a "Form 1823" filled out by a physician on behalf of the senior in advance of touring any facilities (has a 60-day shelf life before a new one is needed). Download Form 1823 (AHCA, 2021).

Facility tours

  • Identify potential eligible facilities that meet the senior's specific needs
  • Schedule appointments with the facilities on behalf of the senior
  • Meet the senior at the short-list of possible facilities, joining them on the tour and interview process with the facility
